Medical Form (PADI / RSTC)
PADI MEDICAL FORM.pdf
This is the 2023 RSTC Approved Medical form for Scuba Diving Activities. It must be filled out before any dive training is begun.
A Physician’s Approval is required for any diver who has a potential or actual medical condition that is deemed adverse to scuba diving—specifically if any question on Page One is answered “Yes,” and Page Two details indicate a physician’s release is required.
